Medigap vs Medicare Advantage
Two ways to limit what Original Medicare leaves you paying.
Medigap and Medicare Advantage both answer the same problem: Original Medicare's open-ended cost sharing. They solve it differently, and you can't have both at once.
Side by side
| Medigap + Original Medicare | Medicare Advantage | |
|---|---|---|
| How it works | Adds to Original Medicare and pays after it | Replaces how you get Parts A and B |
| Doctors | Any provider in the US accepting Medicare | Usually a network |
| Premium | Monthly Medigap premium, plus Part D | Plan premium (varies), usually including drugs |
| Costs when you use care | Low to none, depending on the plan | Copays and coinsurance up to the plan's limit (at most $9,250 in-network in 2026) |
| Prior authorization | Rare | Common |
| Extras | None | Some plans offer dental, vision, hearing |
| Buying later | Health questions possible outside protected windows | Can join any year during Open Enrollment |
The one-way door
You can join Medicare Advantage any year, regardless of health. Going the other way, from Medicare Advantage to Original Medicare with Medigap, may require medical underwriting in most states. That asymmetry is the most important thing to understand before choosing, especially at 65, when your Medigap Open Enrollment Period guarantees you a policy.
Common questions
No. Insurers can't sell you Medigap while you're in a Medicare Advantage plan, unless you're switching back to Original Medicare, and Medigap wouldn't pay anything while you're in an Advantage plan.
Original Medicare with Medigap works with any Medicare provider across the US, and six Medigap plans include some emergency coverage abroad. Many Medicare Advantage plans cover only emergencies and urgent care outside their area.
