Free Medicare counseling in Maryland
Maryland’s State Health Insurance Assistance Program (SHIP) is State Health Insurance Assistance Program (SHIP). Its counselors give free, one-on-one help with enrollment, comparing plans, Medigap, and programs that lower costs. They don’t sell insurance. Read more about what a SHIP counselor can do.
Medigap if you’re under 65
Insurers must offer Plans A and D, without underwriting, to people under 65 on Medicare because of a disability who apply within 6 months of Part B enrollment, with premium limits.
Federal law only guarantees a Medigap Open Enrollment Period to people 65 and older, so state law decides what’s available before then. See Medicare under 65 on disability.
Switching Medigap plans: Maryland's birthday rule
Since July 1, 2023, Maryland gives you your birthday plus the 30 days after it, each year, to switch to a Medigap policy with equal or lesser benefits from any insurer, without health questions.

Help paying Medicare costs in Maryland
Maryland’s Medicaid program (Maryland Medicaid (managed-care program: HealthChoice)) also runs the Medicare Savings Programs, which can pay your Part B premium and, for some people, deductibles and coinsurance. States may set limits higher than the federal minimums, so apply even if your income is a little over them. Qualifying for one also gets you Extra Help with drug costs automatically.
Maryland also has a state pharmaceutical assistance program: Senior Prescription Drug Assistance Program (SPDAP). Ask the SHIP whether you qualify.
