Medicare under 65 on disability
Same Medicare, different timelines, and your state matters more.
People under 65 who qualify for Medicare through a disability get the same Medicare as everyone else. It works the same way, but when it starts, and whether they can buy Medigap, follow different rules.
When coverage starts
- Social Security Disability Insurance (SSDI) or certain Railroad Retirement disability benefits: after 24 months of benefits, enrolled automatically.
- ALS: as soon as disability benefits start, with no waiting period.
- ESRD: separate rules based on dialysis or transplant.
Medigap before 65 depends on your state
Federal law doesn't require insurers to sell Medigap to people under 65. Many states do require it, some for every plan and some for only one or two, and some limit what insurers can charge. Other states have no requirement, and insurers there may not sell to you at all. Check your state's page.
A second chance at 65
When you turn 65, you get a new Initial Enrollment Period and your federal Medigap Open Enrollment Period. Any Part B late enrollment penalty from before 65 ends. It's a good moment to revisit all your choices.
Help paying
Medicaid, the Medicare Savings Programs and Extra Help are especially relevant to people on disability. Some people who return to work may qualify for the QDWI program, which pays the Part A premium.
Common questions
Yes, if you have Part A and Part B and live in the plan's area.
You can generally keep Medicare for a long time after returning to work, and you may be able to buy Part A if premium-free coverage ends. Social Security's work incentive rules apply.
