Life insurance with HIV

This has genuinely changed. A lot of what's written online hasn't caught up.

For years the assumption was that an HIV diagnosis meant life insurance was unavailable. That assumption is now out of date, and repeating it keeps people from applying who could be covered.

A growing number of insurers write fully underwritten life insurance for applicants with well-managed HIV, following updates to major reinsurers' underwriting guidance.

What changed

Underwriting manuals used by insurers and reinsurers have been revised to reflect current clinical reality — that HIV managed with modern antiretroviral treatment is a chronic condition rather than what it was understood to be decades ago.

Swiss Re, one of the largest life reinsurers, updated its guidance for underwriting people living with HIV, and that guidance is used by hundreds of insurers internationally. The practical effect is broader participation and clearer criteria than existed even a few years ago.

What underwriters generally consider

  • Viral load, and whether it is sustainably suppressed
  • CD4 count and its stability over time
  • Consistency with antiretroviral treatment
  • Time since diagnosis and length of stable management
  • Engagement with regular monitoring and follow-up
  • Overall health, including other conditions and risk factors

Carrier selection matters more here than almost anywhere

Insurers have moved at different speeds. Some have updated their guidelines substantially; others have not moved at all. That variation is larger than for most conditions on this site.

Applying to a carrier that hasn't updated its approach can produce a decline that a better-matched carrier wouldn't have issued — and declines are visible to subsequent insurers through industry information exchanges. Approaching the right carrier first genuinely matters.

  • Your treatment history and current regimen
  • Recent viral load and CD4 results
  • Your treating physician or clinic details
  • A record of your monitoring schedule
  • Any other conditions being managed

On disclosure

Applications include health questions and insurers may request records and run laboratory testing. Answer accurately — beyond the practical reality, the contestability period means a misrepresentation can cause a claim to be denied later.
