This shows how a plan's settings divide a bill — it is not a premium estimate and doesn't account for exclusions, waiting periods, or pre-existing conditions, which decide whether a bill is covered at all.

The order the maths happens in

Deductible first, reimbursement rate second, annual limit last. Your deductible comes off the bill, the plan reimburses its percentage of what's left, and the annual limit caps the total it will pay in a policy year.

That order is why a 90% plan with a high deductible can leave you paying more than an 80% plan with a low one.

Watch what happens at the limit

Set the annual limit low and enter a large bill — reimbursement stops dead. For a chronic condition needing treatment across a whole year, the annual limit matters far more than the headline reimbursement rate.

What this can't tell you

Whether the bill is covered at all. Exclusions, waiting periods, and pre-existing condition definitions decide that, and they vary between insurers. This calculator assumes a covered claim.
